2023-24 Health Form Requirements
Please note the following medical record requirements for the 2023-24 school year. Should you have questions, please call the school nurse.
All children in kindergarten, sixth grade and new to the district must provide the school with documentation of a completed health exam within 1 year prior to the start of the school year. All forms must be turned in by October 1st. Download the HEALTH EXAM form here.
All children in 7th and 8th grade who will be participating in an interscholastic sports program must provide the school with documentation of a completed health exam. This form must be updated annually. Download the SPORTS HEALTH EXAM form here https://www.iesa.org/documents/general/IESA-PhysicalCard.pdf
Illinois law requires proof of an eye examination by an optometrist or physician who provides complete eye exams be submitted as a child enters kindergarten or is new to an Illinois school. The exam must be completed within one year prior to your child beginning school. All forms must be turned in by October 1st. Download the EYE EXAM form here.
All children in kindergarten, second and sixth grade must provide the school with documentation of a completed dental exam within 18 months prior to May 15th deadline. All forms must be turned in by May 15th. Download the DENTAL EXAM form here. If you request a waiver from the dental requirement, please use this DENTAL EXAM WAIVER form.

School Supply Sale from PTO
Save time and help support the PTO by ordering school supplies for next school year. Each pack contains exactly what the school supply list requests. Supplies will be shipped directly to your home in August. Shop supplies here
NEW this year: We listened to parent feedback and are offering our 2023-2024 school supply packs from a new vendor! Only top quality, name brand school supplies will be included, and there will be no generic substitutions.

Did you know you have the option to report your student’s absence via an online form? Please use the links below to report an absence before 9:00AM.

Absences can also still be reported via the absence call line if preferred to the online form.
Visitor Procedures
We thank our parents for partnering with us as we reset visitor procedures in the schools. Please note the following updated procedures in place in each school beginning on Thursday, August 25, 2022. We ask that all visitors:
1. enter via the main entrance, buzz at the door, wait to be greeted by a staff member over the intercom, confirm your appointment/meeting
2. if you have a confirmed appointment, please be ready with your license so we may screen with our visitor system
3. please wear your visitor lanyard at all times while in the school for a pre-scheduled appointment
4. return to the main office, return your lanyard, collect your license, and exit via the main entrance
If you are simply dropping a lunch or item for your child, please do so in the vestibule. School staff will be monitoring for items throughout the day and ensure items are brought in for your child.
Our commitment to student safety is unwavering as we embark upon the 2022-23 school year. Thank you for working with our school teams as we implement these new procedures.
